• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge
• Date of Birth

Follow these steps:

1. Using your device, go to this website: matchaffinity.com
2. Log in with email and password
3. Click on the link or button that says “My Account” in the corner
4. Pick “My Subscription”
5. Click on the link or button that says the option to cancel and follow the instructions carefully
6. Be sure to retain your cancellation proof for your records
