• First Name
• Last Name
• Phone Number
• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Using your phone, call (415) 701-1110
2. Let the rep know that you’re calling to cancel
3. Request proof of your transaction in the form of an email or confirmation number
4. Be sure to retain your cancellation proof for your records

Follow these steps:

1. Using your device, go to this website: Including your account information
2. Put in your information where requested
3. Scroll up the page and find Submit
