Get together the following account information:

• First Name
• Last Name
• Phone Number
• Billing Address
• Account Number
• Date of Birth
• Amount of Last Charge
• Password
• Username
• Email Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge

Follow these steps:

1. Get a letter together and request cancellation within it. Provide all of your account info as listed. Sign and date the letter and Keep a copy for your records.
2. Send the letter, making sure to use Certified mail, to the address provided to you by the gym
3. Using your phone, call the phone number for the gym you signed up at or attended most frequently after your letter is received to ensure your account is cancelled
4. Request proof of your transaction in the form of an email or confirmation number
5. Be sure to retain your cancellation proof for your records

2. Ask the rep to explain the cancellation process
3. Give the rep your account info when they ask
4. Ask if you are in a contract or free to cancel
