• Date of Last Charge
• Reason for Cancellation
• First Name
• Last Name
• Phone Number
• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Last 4 Digits of Card
• Amount of Last Charge
• Social Security Number
• Date of Birth

Follow these steps:

1. Using your phone, call 1-800-447-9239
2. Let the rep know that you’re calling to cancel
3. Give the rep your account info when they ask
4. Ask the rep to explain the cancellation process
5. Get a new email together and include your account information
6. Send the letter, making sure to use Certified mail, to PO Box 814530 Dallas, TX 75381
7. Wait 30 Days after receipt
8. Using your phone, call 1-800-447-9239
9. Request proof of your transaction in the form of an email or confirmation number
10. Be sure to retain your cancellation proof for your records
