• First 4 Digits of Card
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Using your phone, call 1-888-983-0264
2. Pick phone menu option 1 and then option 4
3. Let the rep know that you’re calling to cancel
4. Make sure that you won’t be charged again by asking the rep explicitly
5. Request proof of your transaction in the form of an email or confirmation number
6. Be sure to retain your cancellation proof for your records
