• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Get a new email together and request that your account be cancelled
2. Address your email to [email protected]
3. Be sure to retain your cancellation proof for your records
