• First Name
• Last Name
• Phone Number
• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Get a new email together and include your account info as listed
2. Let the rep know that you’re calling to cancel within the email
3. Request proof of your transaction in the form of an email or confirmation number in reply to your email
4. Address your email to [email protected]
5. Be sure to retain your cancellation proof for your records
