• Email Address
• Username
• Password
• Billing Address
• City
• State/Province/Region
• ZIP/Postal Code
• Country
• Reason for Cancellation
• Last 4 Digits of Card
• Date of Last Charge
• Amount of Last Charge

Follow these steps:

1. Using your device, go to this website: https://acorn.tv
2. Log in with username and password
3. Click on the link or button that says “My Acorn TV” in the top right corner
4. Pick “Manage Account”
5. Click on the link or button that says “Cancel Membership” on the left
6. Be sure to retain your cancellation proof for your records
